1. Because we care about your health and wellness, you need to have medical clearance from your doctor using the Rock Hard Fitness/Anchorage Fit Club (RHF/AFC) form so that we may track and assure your well-being.

2. You need to complete an intake, midway, and outtake file to track your results. You also need to meet with your doctor after the program to go over your results. Only those who complete the tracking file will compete for prizes (if prizes are promoted).

3. You must commit yourself to the exercise sessions to compete for prizes. No makeup sessions will be offered.

4. You must use the Isagenix products for the allotted time to compete for prizes. We set you up with an online wholesale associate account.

5. You must commit yourself to the weekly training calls if part of the program. You have to make 22 calls (based on 12 week program) to stay eligible for the prizes (if prizes are promoted).

6. Only those 30 lbs or more overweight may compete for prizes. Others may join the program, but will have to pay regular boot camp prices and will have to comply with all rules and regulations of RHF/AFC. Eligibility will be determined by RHF/AFC and its affiliates.

7. Prize winners (if prizes are promoted) will be judged on total transformation; weight loss, body fat % loss/muscle gain, attendance, diary entries, rule compliance/cooperation, and work ethic.

8. Prize Disclosure: Any prizes promoted may be subject to change based on the number of contestants.

9. You must have a heart rate monitor, yoga mat, and 3 resistance bands (light, medium, and heavy) for off-site workouts.

10. You must have proper attire and running shoes for workouts. You must wear the AFC shirts during workouts (if provided).

11. Payment procedure: For those joining the program, there will be a non-refundable enrollment fee of $299 to assure your place in the program. If you choose to pay more you must adhere to the refund policy in this contract (see section 12 and 14). Any remaining balance will be due no later than 7 days before the formal orientation. You must pay the full balance before the orientation to participate in the program. Late registration fee is $100 (less than 7 days prior to orientation).

12. No refunds will be given once fully enrolled unless you have a medical condition (see Section 14). If you refund any Isagenix products you pay the return shipping, the annual membership/associate fee, plus a $50 AFC administration fee may apply, Signing up and paying full price before any marketing special (early bird, internet) will not necessarily warrant any refund of the difference between full price and special price.

13. If you start the Isagenix program early and become ineligible for any Anchorage Fit Club challenge a refund will be issued. You may apply the refund to boot camp if you choose. Rock Hard Fitness will determine eligibility.

14. Medical Leave: Medical leave must be in writing. A doctor must state in writing that you are unable continue any form of exercise. This must be addressed before the end of Week 2 of the actual workouts to warrant any refund. No refunds will be given after 2 weeks. If you leave the program for a medical reason there is a $350 surrender charge. Any refunds for medical leave will be issued within 30 days.

15. Collection process: In the event that you fail to honor the contract, Rock Hard Fitness reserves the right to initiate collection procedures. Collection procedures involve money owed per the contract, interest at current rate, collection costs, any other applicable fees allowed by law, and all court fees. NSF fee is $25.

16. RHF/AFC reserves the right to change the start date of the program.
